Blood pressure self-monitoring and physician-guided dose changes after birth

Measuring your own blood pressure at home after birth and following an app that suggests changes to blood pressure medicine, with doctors checking the readings and approving the changes. It was tested in one UK trial of women who had had pre-eclampsia or gestational hypertension, and their blood pressure was lower about 9 months after birth than with usual care.

Overview

Technical summary

The strategy tested in POP-HT: a Bluetooth home blood pressure monitor and a smartphone app with a medicine-titration algorithm, with physicians who can review uploaded readings and respond to abnormal readings (registry description). The trial found lower 24-hour blood pressure at about 249 days after birth (diastolic -5.80 mm Hg, systolic -6.51 mm Hg). It is one single-site trial, and its outcome is blood pressure, not heart or blood vessel events. No guideline recommendation on this strategy was read. It links to the core record for home blood pressure monitoring.

About this intervention

Type
Service
Mechanism
The woman measures her own blood pressure with a Bluetooth home monitor and follows a smartphone app that gives advice on changing her medicine dose; physicians review the uploaded readings and approve any change (registry description).

Claims and evidence

Context Women after a pregnancy with pre-eclampsia or gestational hypertension who needed blood pressure medicine at discharge, POP-HT trial, UK

Recipient
Maternal
Population
Women aged 18 years or older whose pregnancy was complicated by pre-eclampsia or gestational hypertension and who needed blood pressure medicine after birth when they were discharged (abstract eligibility). In the trial, 40% had gestational hypertension and 60% had pre-eclampsia, and the mean age was 32.6 years. The outcome is the woman's own blood pressure, so the recipient and the measured population are the same. The registry excludes women with pre-existing hypertension.
Care setting
A single hospital in the UK (abstract). The registry lists two sites, both in Oxford.
Geography
United Kingdom
Life stage
Postnatal period: the primary outcome was measured about 249 days after birth, in the first 9 months postpartum
Gestational age
Not reported
Postnatal age
Not reported
Follow-up
About 9 months after birth (abstract). The 24-hour blood pressure was measured at about 249 days postpartum. The registry gives the primary outcome time frame as 6 months postpartum.
Eligibility: 3 inclusion criteria; 1 exclusion criterion
Included
  • Aged 18 years or older
  • Pregnancy complicated by pre-eclampsia or gestational hypertension
  • Needed blood pressure medicine after birth when discharged
Excluded
  • Women with pre-existing hypertension (registry exclusion criteria); the registry lists other exclusions that are not repeated here
  • Effect Interventional Certainty not assessed

In POP-HT, a randomised trial at a single UK hospital, 220 women who had had pre-eclampsia or gestational hypertension and still needed blood pressure medicine at discharge were assigned to self-monitoring with doctor-guided changes in their medicine (112 women) or to usual postnatal care (108 women). Of them, 200 (91%) were in the primary analysis. At about 249 days after birth, 24-hour mean diastolic blood pressure was 71.2 mm Hg with the intervention and 76.6 mm Hg with usual care, a between-group difference of -5.80 mm Hg (95% CI -7.40 to -4.20; P < .001). Systolic pressure was 114.0 and 120.3 mm Hg, a difference of -6.51 mm Hg (95% CI -8.80 to -4.22; P < .001).

    Certainty method
    Not assessed
    Limitations
The trial was open-label with a blinded end point, run at one site, and the authors call the result an association in a single-centre trial. The outcome is blood pressure, not a heart or blood vessel event; the abstract reports no such events. The registry record was last updated on 2021-10-22 and still lists an estimated enrolment of 200, an estimated primary completion date of 2022-04-01 and a primary outcome time frame of 6 months, against 220 randomised and 9 months in the paper. The two records are kept side by side and are not reconciled. Only the PubMed abstract was read; the PMC copy is not in the open access subset.
