## In plain language

High blood pressure that is still present in the weeks and months after a baby is born. After a pregnancy with pre-eclampsia or gestational hypertension, some women still need blood pressure medicine when they go home, and their blood pressure is checked after birth. The abstract of one UK trial notes that high blood pressure in pregnancy is followed by a higher rate of high blood pressure and heart and blood vessel disease later in life.

## Technical summary

The clearest case in this atlas is the population of a UK trial of home monitoring: women aged 18 or older after a pregnancy complicated by pre-eclampsia (60%) or gestational hypertension (40%), still needing blood pressure medicine when discharged. In that trial self-monitoring with physician-guided medicine changes was followed by lower blood pressure at about 9 months (see the intervention page). The trial abstract states that pregnancy hypertension results in adverse cardiac remodelling and a higher incidence of hypertension and cardiovascular disease later in life; it gives no figures for that statement. The postnatal care page records a recommendation to measure every postpartum woman's blood pressure shortly after birth. This page makes no statement about thresholds or targets, which no source read here gives. It links to the core record for peripartum cardiomyopathy. The trial excluded women with pre-existing hypertension (registry), so it says nothing about chronic hypertension, and no source read here covers new-onset high blood pressure after birth.
