## In plain language

One combined measure of serious problems in the pregnancy: pre-eclampsia with severe features, a medically needed early birth before 35 weeks, the placenta separating from the womb wall before the birth and leading to delivery, or the death of the fetus or newborn. It mixes events for the mother with events for the baby, so it is not split by who has the problem.

## Technical summary

The CHAP definition only. Other trials and guidelines combine events differently. The recipient is mixed and its findings are never split by component or summed with other outcomes. Time windows differ between sources: the manuscript counts pre-eclampsia up to 2 weeks after birth and lists fetal or neonatal death at under 28 days, while the registry gives up to 90 days for neonatal death. The paper follows patients until 6 weeks after birth, and each component has its own event window.
